‘H’ Delivered to location at 17:30. Successful transition. Social Worker provided with de-brief throughout journey, ‘H’ provides electronic feedback to team.
Report with Local Authority EDT with handover information to new placement by 18:00.
On Monday morning the Placement Manager requests 2:1 outreach support for 8 hours daily support for ‘S’, a complex Young Person, high risk of missing episodes, CSE and Self harm.
Proposal sent by SH-SS team within two hours.
Planning meeting on Tuesday morning.
Introduction to outreach team takes place Tuesday afternoon. Outreach support commences Wednesday.
Daily reports submitted to Social Worker and Team around the Child. SH-SS Team Manager attends review meetings.
‘M’ has been admitted to Blackpool hospital, there is concern for her mental health and wellbeing. Her placement have given immediate notice.
SH-SS work with Social Care and Health professionals to co-ordinate a wrap around 24 hour service to engage ‘M’ and alleviate pressure on the Health Service for the duration of her stay. This is reviewed every 48 hours.
‘M’ is supported by staff to engage in activities, have appropriate contact as risk assessed and supported to transition from the current setting inline with care planning.
